Job summary

UnitedHealth Group is seeking a licensed Physical Therapist to provide skilled PT services to patients in their homes, promoting mobility, function and safety while supporting recovery in the home setting. You will collaborate with patients, caregivers and an interdisciplinary care team to improve quality of life.

Weekend schedule: Friday, Saturday and Sunday. Requirements include a CAPTE-accredited PT program, current PT license for the state, CPR certification, and 1+ year of home health

Qualifications

• Current license to practice physical therapy in the state you’re assigned to.
• Graduate from a CAPTE-accredited physical therapy program.
• 1+ year of physical therapy experience in home health care or related field.
• If less than one year, regional clinical leaders’ approval is required and/or residency participation.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations (every 30 days at minimum) and modify plans as needed.
• Perform discharge planning and coordinate with the care team.
• Other duties as assigned.
