Job summary

UnitedHealth Group is seeking a Per Diem Physical Therapist to provide skilled PT services in patients' homes, promoting mobility and independence. You will collaborate with patients, caregivers, and an interdisciplinary team to support recovery and prevent hospitalizations.

Primary duties include maintaining movement, delivering interventions with regular reexaminations, and discharge planning. A CAPTE-accredited program, active state license, CPR, and a valid driver's license are required.

Qualifications

Graduate from a CAPTE-accredited physical therapy program.
Current license to practice physical therapy in the assigned state.
CPR certification is current.
1+ years of related PT experience for home health care.
If less than one year, regional clinical leaders' approval and mentoring/residency may be needed.
Driver's License and reliable transportation.

Responsibilities

Help patients maintain, restore and improve movement, activity and function.
Provide interventions, conduct reexaminations at least every 30 days, modify interventions, and perform discharge planning.
Other duties as assigned.
