Job summary

UnitedHealth Group is seeking a Per Diem Physical Therapist to deliver skilled home health PT services, promoting movement, safety and independence in patients' homes. You will work with patients, families and the care team to support recovery and prevent hospitalizations.

Responsibilities include examinations every 30 days, plan of care modifications as needed, and discharge planning; additional duties may be assigned. Prior home health experience and CAPTE-accredited education are preferred.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from an accredited physical therapy educational program per CAPTE.
• 1+ year of physical therapy experience in home health care or equivalent.
• If <1 year, regional mentor approval and/or residency participation may be required.
• Current CPR certification.

Responsibilities

• Provide skilled physical therapy services to patients in their homes to promote mobility and independence.
• Collaborate with patients, caregivers and interdisciplinary care team to support recovery and prevent hospitalizations.
• Conduct reexaminations at least every 30 days, modify interventions as needed and perform discharge planning.
• Perform other duties as assigned.
