Job summary

PACS seeks a dedicated caregiver to provide routine daily care and services to residents in a supportive environment. You will follow each resident’s care plan, assist with daily activities, and maintain a warm, respectful atmosphere on the unit.

Responsibilities include accurately documenting care, reporting changes to supervisors, and ensuring resident safety and comfort throughout shifts. A high school diploma is required, and proficiency with Microsoft Office is expected.

Qualifications

• Must possess, as a minimum, a high school education or its equivalent.

Responsibilities

• Provide routine daily care and services per resident assessment and care plan.
• Report changes in residents' condition to Health and Wellness Director promptly.
• Document entries in notes, SPAs, and incident reports in a clear, descriptive manner.
• Maintain confidentiality of resident information and report any unauthorized disclosures.
• Assist residents with daily living activities and mobility as trained.
• Keep residents comfortable, greet them, and escort to rooms.
• Support with beds, bedding, clothing, and personal possessions.
• Observe and report skin issues, injuries, or changes in condition.
• Follow policies, procedures, and shift tasks; participate in nursing reports.
