Job summary

PACS is seeking a dedicated caregiver to provide routine daily care and services to residents per established care plans. The role requires timely reporting of any changes in residents’ condition to the Health and Wellness Director and maintaining strict confidentiality of health information.

You will perform various daily tasks to assist residents, including personal care, mobility support, and documentation; collaborating with nursing staff to ensure high quality care and safety across shifts.

Qualifications

• High school education or equivalent is required.

Responsibilities

• Provide routine daily care and services to residents per care plan.
• Report changes in residents' condition to the Health and Wellness Director.
• Maintain confidentiality of residents' protected health information.
• Assist with activities of daily living and mobility as trained.
• Record entries accurately in notes and incident reports.
