Job summary

PACS is seeking a compassionate caregiver to provide routine daily care and services to residents in a post-acute setting, following each resident’s assessment and care plan.

You will document care activities, report changes to nursing leadership, and protect resident privacy. You will assist with meals, mobility, and personal care, while maintaining a warm, safe environment and collaborating with the care team. Must have a high school diploma and basic Microsoft Office skills.

Qualifications

• High school diploma or equivalent required.
• Proficient in Microsoft Office suite.
• Ability to read, analyze, and interpret general business materials.

Responsibilities

• Provide routine daily care and services per resident care plan.
• Use PCC to ensure correct resident information and report changes to nursing leadership.
• Record entries and report accidents/incidents; maintain confidentiality of resident information.
• Assist with meals, transfers, mobility, and daily personal care as trained.
• Maintain a clean, safe, and comfortable environment; communicate with residents and staff.
