Job summary

Residential Transitions, Inc. (RTI) seeks a Program Manager for Waiver Case Management in Ramsey County.

You will lead a team providing person-centered case management for CADI/BI waiver clients, coordinate client intake and discharges, and collaborate with medical and mental health professionals to craft individualized plans. The role requires 2+ years of management experience, a relevant degree, travel in the Metro area, and a strong grasp of community resources.

Qualifications

Must meet any of the three qualifications (social work/psych/sociology degree with 2000 hours in case management, or RN with case management licensing).
Minimum 2 years of management experience.
Waivered Case Management background preferred.
Knowledge of community resources and how to access them.
Willingness to travel within the Metro Area with a valid driver's license and auto insurance.

Responsibilities

Lead, supervise, and develop the team to support business needs, including engagement, motivation, and retention.
Manage a caseload of individuals with serious mental illness, substance use disorders, and co‑occurring conditions.
Visit clients monthly and twice yearly per waiver rules; coordinate intake and discharges.
Collaborate with physicians, mental health providers, and other teams to update care plans.
Ensure financial authorizations and provider contracts are in place; document services in records.
