Job summary

Residential Transitions, Inc. (RTI) is seeking a Waiver Case Manager in Ramsey County, MN. You will provide person-centered case management to individuals with serious mental illness and co-occurring conditions, with training at the main office for 60 days.

Waiver Case Manager

Company Overview

Position Overview

RTI is seeking a dedicated Waiver Case Manager for Ramsey County. This role involves providing person‑centered case management services to individuals diagnosed with serious and persistent mental illness, substance use disorders, and co‑occurring medical conditions. Candidates will receive in‑person training at our main office for the first 60 days.

Role Responsibilities
  • Maintain and manage a caseload of individuals across Ramsey County.
  • Visit clients at least once a month and twice annually in accordance with waiver‑specific rules.
  • Coordinate client intake and discharges within set parameters and timelines.
  • Collaborate with physicians, mental health providers, and other teams to develop and update client plans.
  • Ensure financial authorizations and provider contracts are in place.
  • Document services in individual records to meet record‑keeping standards and evaluate outcomes.
  • Access community and housing resources to match client needs and preferences.
  • Provide crisis assessment and intervention to keep clients safe in their homes and communities.
Required Qualifications and Experience
  • Qualified under Minnesota law: graduate of an accredited four‑year college with a major in social work, psychology, sociology, or a closely related field; or graduate of any field with one year of experience as a social worker, case manager, or care coordinator in a public or private social service agency.
  • Willingness to travel within the Metro Area; current driver’s license, clean record, personal vehicle with insurance; ability to transport clients.
  • Must not be actively receiving services from RTI or have received services within the last two years.
Preferred Qualifications and Experience
  • Master’s degree in behavioral science or a related field.
  • Experience providing direct services to adults with serious and persistent mental illness.
  • Experience with case management and commitment proceedings.

Physical & Mental Requirements
  • Walk, stand, sit, bend, climb stairs, and perform crisis prevention/intervention techniques.
  • Use hands for writing, typing, and data entry.
  • Lift up to 40 lbs. and sometimes exert physical effort during duties.
  • Maintain vision and hearing sufficient for reading charts, operating office equipment, and using a computer.
