Job summary

Residential Transitions, Inc. (RTI) is hiring a Program Manager for Waiver Case Management in Ramsey County, MN. You will lead a team delivering person-centered services under CADI/BI waivers, coordinating client intake, plans, and discharges while ensuring compliance and resource access.

The role requires 2+ years of management, a relevant degree or RN/public health license, and travel within the metro area. RTI offers PTO, benefits, and growth opportunities.

Qualifications

• Must meet one of three qualifications: degree in social work/psychology/sociology; or degree in any field with 2000 hours social work experience; or licensed RN/public health professional under Minn. Stat. 148.171-148.285.
• Two years of management experience required.
• Knowledge of community resources and how to access them.

Responsibilities

• Lead, supervise, and develop the team to support business needs.
• Manage and maintain a caseload of individuals with serious mental illness and co-occurring conditions.
• Visit clients at least monthly and twice annually per waiver rules.
• Coordinate client intake and discharges within set parameters.
• Collaborate with physicians, mental health providers, and teams to develop/update plans.
• Ensure financial authorizations and provider contracts are in place.
• Document services in individual records and maintain records.
• Access community/housing resources matching needs.
• Provide crisis assessment and intervention for safety.
