Job summary

Residential Transitions, Inc. (RTI) in Ramsey County, MN is hiring a Program Manager for Waiver Case Management. You will lead a team serving individuals with serious mental illness and co-occurring conditions, ensuring coordinated services and timely waivers.

The role requires supervision experience, strong resource knowledge, and willingness to travel within the Metro Area. RTI emphasizes dignity, respect, and person-centered care across Minnesota.

Qualifications

• Bachelor’s degree in social work, psychology, sociology or a closely related field.
• Two years of management experience.
• Experience with Waivered Case Management.
• Knowledge of community resources and how to access them.

Responsibilities

• Lead, supervise, and develop the team to support business needs through engagement, motivation, and retention.
• Manage a caseload of individuals with mental illness and co-occurring conditions across Dakota County.
• Visit clients monthly and twice annually per waiver rules; coordinate intake and discharges.
• Collaborate with physicians, mental health providers, and teams to update client plans.
• Ensure financial authorizations and provider contracts are in place and documented records are maintained.
