Job summary

Residential Transitions is seeking a Program Manager-Waiver Case Management in Minnesota. The role leads a team delivering person-centered case management for CADI/BI waiver clients, coordinating intake, discharges and care plans with health professionals.

You will supervise staff, ensure contracts and authorizations, and support access to housing and community resources. Strong driving record and willingness to travel in the metro area are required.

Qualifications

• Must meet degree or licensure options as described by the employer.
• Two years of management experience is required.
• Experience with Waivered Case Management preferred.

Responsibilities

• Lead, supervise, and develop a team to meet business needs.
• Manage a caseload for individuals with SMI and co-occurring conditions.
• Coordinate client intake/discharges within timelines.
• Collaborate with physicians and mental health providers to plan care.
• Ensure financial authorizations and provider contracts are in place.
• Document services in records and maintain reporting standards.
• Access community resources matched to client needs.
