Job summary

Residential Transitions, INC. is seeking a Program Manager for Waiver Case Management to lead a team serving individuals with serious mental illness and co-occurring conditions across Dakota county in Minnesota.

You will supervise staff, coordinate client intake and discharges, and ensure high-quality documentation and financial authorizations. The role requires two years of management experience, knowledge of Waivered Case Management, and willingness to travel within the Metro Area.

Qualifications

• Must meet any of the three qualifications: BA/BS in social work/related field, or 2000 hours as social worker/case manager, or licensed public health/registered nurse with case management.
• Two+ years of management experience.
• Background in Waivered Case Management.

Responsibilities

• Lead, supervise, and develop the team to support business needs through engagement, motivation, and retention.
• Manage caseloads of individuals with serious mental illness, substance use disorders, and co-occurring conditions.
• Visit clients at least monthly and twice annually per waiver rules.
• Coordinate client intake and discharges within timelines.
• Collaborate with physicians and mental health providers to update client plans.
• Ensure finances, contracts, and documentation meet standards.
• Access community and housing resources matching client needs.
• Provide crisis assessment and intervention when needed.
