Job summary

Residential Transitions, Inc. (RTI) seeks a Program Manager for Waiver Case Management to lead a team serving individuals with serious mental illness and co-occurring conditions in CADI/BI waivers in Dakota and Hennepin counties.

You will supervise staff, coordinate intake, and ensure documentation, authorizations, and contracts are in place with a person-centered approach. The role requires a four-year degree in a related field or equivalent, plus at least two years of management experience,

Qualifications

• Must meet any of the three qualifications: a four-year degree in social work/psychology/sociology or related field; or a four-year degree in any field with 2000 hours of relevant case management experience; or a licensed public health or registered nurse practicing case management.

Responsibilities

• Lead, supervise, and develop the team to support business needs through engagement, motivation, and retention.
• Manage and maintain a caseload of individuals with serious mental illness and related conditions.
• Visit clients monthly and twice yearly per waiver rules.
• Coordinate client intake and discharges within set parameters and timelines.
• Collaborate with physicians and mental health providers to update client plans.
• Ensure financial authorizations and provider contracts are in place.
• Document services in records to meet standards.
