Job summary

Benchmark Human Services-VT is seeking Case Managers to support individuals with intellectual and developmental disabilities. The role involves providing resources, conducting assessments, and facilitating person-centered planning sessions.

The ideal candidates must have a valid Driver's License, knowledge of Vermont's public services, and experience in human services. Benefits include health insurance, a 401k plan, profit sharing, and more.

Qualifications

• Valid Driver's License.
• Valid Auto Insurance.
• Must live in the state of VT.
• Minimum of 1 year working in a similar position.
• Travel up to 75%.

Responsibilities

• Complete core competency training required by the State.
• Perform evaluations and assessments to meet the needs of individuals served.
• Assist individuals to understand the goals of the person-centered planning process.
• Monitor person-centered service plan implementation through visits and documentation.

Knowledge of Vermont’s public service system for developmental disabilities services

Communication skills with a broad audience

Education

Bachelor’s degree in any field + 1 year of human services related experience

Associate degree in human services + 2 years of related experience

High school diploma + 3 years of human services related experience
