Job summary

Benchmark Human Services is hiring Case Managers in Vermont to support individuals with intellectual and developmental disabilities, their families, and guardians. You will help plan, access, coordinate, and oversee services to promote maximum independence and community inclusion.

Qualifications

• Knowledge of Vermont’s public service system for developmental disabilities and brain injury services.
• Ability to communicate effectively with diverse stakeholders.
• Ability to work collaboratively while meeting individual performance goals.

Responsibilities

• Perform evaluations and assessments to meet the needs of individuals served.
• Assist individuals in understanding goals and activities of person-centered planning.
• Facilitate and schedule participation in person-centered planning and assessments.
• Complete Needs Assessment Annual Review Protocol with the individual and team.
• Assist in discovery discussions to identify preferences, strengths, goals and needs.
• Assist in preparing for the person-centered planning process and related documents.
• Support the individual to lead the person-centered planning process as needed.
• Review and update the person-centered service plan and related documents.
• Monitor plan implementation, satisfaction, and wellbeing through visits and guardian/family input.
• Respond to emergencies and incidents to protect health and welfare.
