Job summary

Benchmark Human Services-VT is looking for dedicated Case Managers to provide support for individuals with intellectual and developmental disabilities in Bennington, VT. The ideal candidates will actively participate in helping individuals reach their full potential through person-centered planning and assessments.

A bachelor's degree in a relevant field and experience in human services are required. The role involves travel and has a strong commitment to fostering independence and inclusion.

Qualifications

• Minimum of 1 year working in a similar position.
• Travel up to 75%.
• Valid Driver's License and Auto Insurance.

Responsibilities

• Complete core competency training required by the State.
• Perform evaluations and assessments to meet the needs of individuals served.
• Facilitate and schedule participation in person-centered planning.

Skills

Knowledge of Vermont's public service system

Collaborative working

Education

Bachelor's degree + 1 year experience

Associate degree + 2 years experience

High school diploma + 3 years experience
