Benchmark Human Services in Vermont is hiring Case Managers to support individuals with intellectual and developmental disabilities and their families. The role focuses on planning, advocacy, monitoring, and coordinating services to enable inclusive community participation and independence.

The program emphasizes person-centered planning, collaboration with guardians and support teams, and documentation of planning outcomes.

Formación

• Knowledge of Vermont’s public service system for developmental disabilities and brain injury services.
• Ability to work collaboratively while meeting individual performance goals.
• Strong communication with a broad audience of stakeholders.

Responsabilidades

• Complete core competency training required by the State.
• Perform evaluations and assessments to meet the needs of individuals served.
• Support person-centered planning processes and scheduling of planning meetings.
• Assist individuals in discovery, exploring preferences, goals, and needs.
• Review and update person-centered service plans and related documents.
• Monitor service plan implementation and health and welfare through visits and guardian engagement.
• Respond to emergencies and incidents to protect health and welfare.
