Job summary

Benchmark Human Services is hiring a Vermont-based Case Manager to support individuals with intellectual and developmental disabilities, coordinating planning, services, and resources to promote independent living and community inclusion.

You will work closely with individuals, families, and guardians, guiding person-centered planning, assessments, and service plans while ensuring compliance with state requirements and coordinating across teams.

Qualifications

• Knowledge of Vermont’s public service system for developmental disabilities and brain injury services.
• Must be able to verbally and non–verbally communicate with a broad audience of stakeholders.
• Ability and skill to use technology tools in the daily performance of duties.

Responsibilities

• Complete core competency training required by the State.
• Perform evaluations and assessments to meet the needs of individuals served.
• Assist individuals and families to understand and participate in person-centered planning.
• Facilitate and schedule participation in person-centered planning and assessment.
• Complete annual Needs Assessment Review with the individual and team.
• Assist in discovery of preferences, strengths, goals, and needs for planning.
• Support completion of the DS Budget and Service Planning Context Document.
• Coordinate gathering of documentation for planning processes.
• Help assemble the planning team and invite participants as appropriate.
• Draft agendas and follow-up notes for planning meetings and distribute to the team.
• Help the individual lead the planning process with support as needed.
• Identify modifications to rights and freedoms as part of planning.
• Review and update the person-centered service plan and related documents.
• Monitor plan implementation, satisfaction, and well‑being with visits and records.

Bachelor’s degree + 1 year human services

Associate degree + 2 years human services

High school diploma + 3 years human services experience
