Job summary

Benchmark Human Services-VT is seeking dedicated Case Managers to support individuals with intellectual and developmental disabilities (IDD) and their families in planning, coordinating, and accessing services. You will lead person-centered planning, assessments, and advocacy to help individuals live as independently as possible in their communities across Vermont.

Qualifications

• Bachelor's degree in any field + 1 year of human services related experience.
• Associate degree in human services + 2 years of human services related experience.
• High school diploma or equivalent + 3 years of human services related experience.

Responsibilities

• Complete Needs Assessment Annual Review Protocol with the individual and their team.
• Assist and support the individual to understand the goals and activities of person-centered planning.
• Facilitate and schedule participation in person-centered planning and assessment as needed.
• Review and update person-centered service plan and related documents with the individual.
• Monitor plan implementation, satisfaction, and wellbeing through visits and reviews with guardians/family.
