Benchmark Human Services is seeking a Case Manager in the Springfield/White River Junction area of Vermont. You will support individuals with intellectual and developmental disabilities through planning, advocacy, and coordination of services to promote independence and community inclusion.

Responsibilities include assessments, person-centered planning, and collaboration with families, guardians, and service providers.

Qualifikationen

• Knowledge of Vermont's public service system for developmental disabilities and brain injury services.
• Ability to communicate effectively with diverse stakeholders both verbally and non-verbally.
• Proficiency with technology tools used daily in service planning and documentation.

Aufgaben

• Perform evaluations and assessments to meet the needs of individuals served.
• Assist individuals and families with person-centered planning and assessment processes.
• Facilitate planning meetings and coordinate schedules with the support team.
• Complete annual Needs Assessment and Service Planning documentation.
• Support individuals in choosing planning team members and coordinating participation.
• Monitor implementation of service plans and review related documents with the individual and guardians.
