• Develop and monitor personalized care plans and coordinate with multi-disciplinary resources.
• Engage members by phone, digital, or virtual visits to improve health outcomes.
• Document activities in compliance with regulations and program standards.
• Participate in workgroups to improve program efficiency and member focus.
