Highmark Health seeks a seasoned wellness professional to oversee a panel of members, delivering personalized health coaching and care plans. You will work with a multidisciplinary team, using data to guide interventions and track outcomes across health statuses and regions.

Telephonic, digital, or virtual engagement supports member-centered care, with a focus on outcomes and cost savings. Requirements include a Master’s in Nutrition or related field and 3 years in wellness/lifestyle coaching.

Qualifications

• Master's Degree in Nutrition or related field required.
• 3 years as RD/EP/Wellness/Lifestyle Coach in a wellness-focused environment.

Responsibilities

• Deliver lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity, and maternal health.
• Develop personalized care plans addressing identified needs, remove barriers, identify resources, and set short/long-term goals.
