Highmark Inc. seeks a seasoned wellness coach to manage a panel of members with varying health needs, delivering personalized care plans and coordinating with a multidisciplinary team.

You will conduct outreach telephonically or digitally, monitor outcomes, and document activities in compliance with HIPAA and company policies. Requirements include a Master’s degree and 3 years of experience as a RD, EP, or wellness coach, with preferred certifications in tobacco cessation, diabetes education, or

Qualifications

• Master's degree or equivalent in a health-related field as stated by the company in lieu of master’s degree
• 3 years as a Registered Dietician OR Exercise Physiologist OR Wellness/Lifestyle Coach in a wellness-focused environment

Responsabilités

• Maintain knowledge of evidence-based interventions for tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health.
• Oversee a specified panel of members by assessing health management needs and triaging to appropriate resources.
• Create care plans with long- and short-term goals and regular contact for progress reassessment.
• Demonstrate clinical outcomes and cost savings from wellness interventions.
• Ensure all activities are documented and compliant with policies and regulatory requirements.
• Participate in workgroups to improve efficiency and member-centric wellness services and develop new programs.
• Other duties as assigned or requested.
