Job summary

Highmark Health is seeking a wellness coaching professional to own a panel of members, assessing health management needs and coordinating interventions. You will design personalized care plans, address barriers, and drive engagement through telephonic and digital outreach.

You will monitor quality outcomes, collaborate with a multidisciplinary team, and ensure compliance with policies while contributing to program development and pilots.

Qualifications

• Master's Degree or equivalent experience in a relevant field.

Responsibilities

• Maintain knowledge of evidence-based interventions and industry standards to deliver high quality lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health.
• Maintain oversight over a specified panel of members by assessing health management needs and triaging to resources.
• Create care plans with long- and short-term goals and regular contact for reassessment.
• Demonstrate clinical outcomes and cost savings from wellness interventions across diverse member groups.
• Ensure activities are documented and compliant with requirements and standards.
• Participate in workgroups to improve efficiency and effectiveness of wellness coaching services.
• Other duties as assigned.
