Job summary

Highmark Health is seeking a Wellness Coach to oversee a panel of members with varying health needs. You will assess health management requirements and develop personalized care plans, coordinating with a multidisciplinary team to drive improved health outcomes.

You will engage with members remotely or via telehealth, monitor progress, and ensure compliance with clinical, financial, and regulatory standards while contributing to program development and new pilots.

Qualifications

• 3 years as a Registered Dietitian OR Exercise Physiologist OR Wellness/Lifestyle Coach in a wellness-focused environment.
• Master's degree OR equivalent experience as determined by the company in lieu of master’s degree.
• Knowledge of evidence-based health education practices and wellness interventions.

Responsibilities

• Maintain knowledge of evidence-based interventions and industry standards to deliver high quality lifestyle coaching.
• Oversee specified panel of members by assessing health management needs and triaging to resources.
• Create care plans with long- and short-term goals and regular contact for reassessment.
• Demonstrate clinical outcomes and cost savings from wellness interventions across diverse member groups.
• Ensure activities comply with regulatory and accreditation standards.
• Participate in workgroups to improve efficiency and member-centric wellness services.
• Perform other duties as assigned.
