Highmark Health seeks a Wellness/Lifestyle Coach to oversee a panel of members, assessing health management needs and designing personalized care plans. You will coordinate with a multi-disciplinary team and use data analytics and clinical judgment to select interventions, while guiding members through telephonic, digital, or virtual interactions to improve outcomes.

You will document activities in compliance with regulatory standards and contribute to program improvements through participation

Competenze

Master's degree in nutrition, health education, health promotion, exercise science, public health, social work/psychology, or related field, or equivalent experience.
Minimum 3 years as a registered dietitian, exercise physiologist, or wellness/lifestyle coach in a wellness-focused environment.

Mansioni

Deliver lifestyle coaching across tobacco cessation, weight management, sleep, nutrition, and activity.
Oversee a panel; assess health needs and triage to resources.
Create and modify care plans with goals and follow-ups.
Show clinical outcomes and cost savings from wellness interventions.
Document activities in compliance with policies and standards.
Join workgroups to improve program efficiency and member focus.
Perform additional duties as assigned.
