Job summary

Highmark Health is seeking a wellness professional to manage a panel of members, assess health needs and develop personalized care plans. You will coordinate with a multi-disciplinary team, implement evidence-based interventions, and track outcomes to improve health metrics.

The role requires a Master’s degree or equivalent experience, 3+ years in wellness coaching, and strong communication skills. Travel and HIPAA compliance are part of the responsibilities.

Qualifications

• Master's degree required or relevant experience as determined by the company
• 3+ years as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused environment

Responsibilities

• Maintain knowledge of evidence-based interventions and industry standards to deliver high-quality lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress, nutrition, physical activity and maternal health.
• Oversee a specified panel of members by assessing health management needs and triaging to resources; create care plans with long- and short-term goals.
• Demonstrate clinical outcomes and cost savings from wellness interventions across age groups and regions.
• Ensure all activities are documented in compliance with applicable requirements and standards.
• Participate in workgroups to improve efficiency and member-centric wellness coaching services.
