Job summary

Fresenius Medical Care is seeking a Kidney Care Specialist to educate CKD/ESRD patients, navigate care, and engage across field and virtual settings. The role involves collaboration with nephrologists and care teams to ensure timely education and referrals.

You will document interactions in designated systems, track engagement, and contribute to continuous improvement of virtual education materials. A valid driver's license and ability to travel within the market are required.

Qualifications

Strong communications skills with the ability to explain complex clinical concepts clearly.
Experience delivering virtual or telephonic patient education.
Proficiency with Microsoft Office and CRM/EMR platforms.

Responsibilities

Provide virtual care navigation to CKD and ESRD patients using digital platforms.
Conduct scheduled and on-demand virtual or in-person patient interactions to assess needs and barriers.
Collaborate with nephrologists and care teams to support referrals and coordinated patient care.
Build relationships with nephrologists, clinicians, and staff to support value-based care objectives.
Ensure timely communication and education updates to referring practices and patients.
