Job summary

Fresenius Medical Care is seeking a Kidney Care Specialist to educate CKD/ESRD patients, navigate care, and engage families across a field-based role. You will travel locally within your assigned territory to meet nephrology providers and patients, delivering education and coordinating care with the care team.

The role emphasizes patient advocacy, relationship-building, and using virtual tools and CRM/EMR systems to track progress and outcomes.

Qualifications

• 1–3 years of experience in kidney care or chronic disease management.
• Experience with CKD/ESRD populations preferred.
• Experience delivering virtual or telephonic patient education preferred.
• Strong communication and remote engagement skills.

Responsibilities

• Provide virtual or in-person patient education and navigation.
• Collaborate with nephrologists and care teams to coordinate care.
• Track patient engagement and outcomes using CRM and EMR systems.
• Support referral workflows and patient follow-through.
• Maintain up-to-date knowledge of kidney care programs.
