Job summary

Fresenius Kidney Care is seeking a dedicated Kidney Care Advocate to provide virtual navigation and education to CKD/ESRD patients. You will collaborate with nephrologists and care teams to support referrals, coordinate care, and document interactions in Salesforce/EMR.

Qualifications

• High School diploma or GED is required.
• 1–3 years in kidney care or chronic disease management preferred.
• Experience with CKD/ESRD/dialysis or chronic disease populations preferred.
• Experience delivering virtual or telephonic patient education strongly preferred.
• Excellent verbal and written communication; ability to explain complex concepts clearly.

Responsibilities

• Provide virtual care navigation to CKD and ESRD patients regarding disease progression, treatment options, home modalities, transplantation, and self-management.
• Conduct scheduled and on-demand virtual or in-person patient interactions via video, phone, and electronic communication.
• Assess patient needs, readiness, barriers, and preferences.
• Collaborate with nephrologists, care teams, and internal partners to support coordinated care and appropriate referrals.
• Build and maintain relationships with nephrology and practice staff to support Kidney Care Advocate and value-based care objectives.
• Coordinate with internal stakeholders to ensure consistent messaging, workflows, and patient follow-through.
• Serve as a resource to referring practices through timely communication, education updates, and patient status reporting.
• Document patient interactions, education, and outcomes in Salesforce, EMR, or other approved systems.
• Support referral workflows and track patients across the kidney care continuum.
• Use virtual tools, dashboards, and reports to track engagement, outcomes, and program performance.
• Participate in development, evaluation, and improvement of virtual education materials and content.
• Maintain current knowledge of kidney disease, treatment modalities, and Fresenius Kidney Care programs.
• Identify barriers to care and connect patients with appropriate resources.
• Support quality, safety, compliance, regulatory, privacy, and documentation standards.
• Participate in team meetings, training, and performance reviews.
• Support patients longitudinally by reinforcing education and navigating next steps.
• Travel extensively to physician groups, healthcare locations, regional business units, corporate meetings, training, and associate home base.
• Perform other related duties as assigned.
