Job summary

Qualifications

• 1–3 years of kidney care, chronic disease management, or related healthcare fields preferred.
• Experience delivering virtual or telephonic patient education is strongly preferred.

Responsibilities

• Provide virtual care navigation to CKD/ESRD patients and educate on treatment options.
• Collaborate with nephrologists, care teams, and internal partners to support referrals.
• Document patient interactions and outcomes in designated systems.
• Track patient engagement and program performance using dashboards and reports.
