Job summary

Mercy seeks a Nurse Manager for the Medical Management & Transition Team to guide operational, clinical, and strategic directions for virtual-based transitional care programs. You will oversee ED-to-Home-accelerated outpatient management, chronic condition follow-up, and intensive care coordination, ensuring safety and high-quality patient-centered care.

You will lead a multidisciplinary team, drive performance improvement, manage budgets, and promote population health strategies within Mercy's

Qualifications

• Graduate of an approved school of nursing; RN licensure required.
• BSN or in-progress toward BSN/BS/MS preferred; may complete in 2 years.
• Four years of clinical experience with supervisory/management exposure preferred.
• CPR and NRP certifications required.
• Must pass medication and math competency test prior to medication administration.
• Basic computer skills including data entry, Windows, and MS Office.

Responsibilities

• Provide operational, clinical, and strategic leadership for virtual transitional care programs.
• Ensure safe, effective, patient-centered care delivery and high-quality outcomes.
• Oversee daily operations, staff performance, and regulatory compliance.
