Job summary

Catholic Community Services is seeking a Case Manager for the Veterans Services program serving nine counties. You will manage a caseload in the Veterans Homeless Prevention/Rapid Re-housing program, perform intakes, assess housing needs, and assist with financial housing costs.

You will develop Housing Stability Plans with VA/community resources, enter data into HMIS, and coordinate with the Veterans Program Manager to deliver strong case management. Driving may be required.

Qualifications

• Post-secondary education in social services field or 2 years relevant experience.

Responsibilities

• Complete thorough intakes and eligibility screening with referred clients.
• Initiate Housing Stability Plan.
• Provide Outreach to potential eligible Veterans.
• Provide data entry.
• Update and carry out client Housing Stability Plan.
• Maintain records and reports.
• Attend meetings as necessary and represent CCS Veteran Services Program.
• Other duties as assigned.
