Job summary

S:US is seeking a Service Coordinator to serve as the primary S:US case manager for veterans and families in Rapid Rehousing or Homelessness Prevention services. You will monitor progress, develop Housing Stability Plans, and coordinate services to improve housing stability.

Responsibilities include field visits, assisting with access to healthcare and benefits, and collaborating with the SSVF team to deliver comprehensive support while maintaining confidentiality and compliance.

Responsibilities

• Provide resources, core SSVF services and referrals to a caseload.
• Navigate veterans through complex human services systems and benefits.
• Deliver person-centered case management using progressive engagement.
• Develop and monitor Housing Stability Plans with SSVF staff.
• Meet participants in the field, VA sites, shelters, or residences (up to 40% fieldwork).
• Maintain relationships with colleagues and program managers.
• Respond to participant needs promptly and professionally.
• Coordinate Housing First-oriented Rapid Rehousing and Homelessness Prevention services.
