Job summary

Insight Housing is seeking a Case Manager to deliver housing stabilization services to Homeless Veterans and their families. Under supervision, you will coordinate intake, assess eligibility, and maintain client files while connecting participants to VA benefits and other resources.

The role emphasizes outreach, crisis intervention, and service coordination to improve health and housing stability, with HMIS data management and ongoing collaboration with the Housing Specialist.

Qualifications

• High school diploma required; bachelor’s degree in social services or related field is a plus.
• CPR/First Aid training required.
• 1 year of direct support experience with homelessness, mental health, or related issues.
• Proficient in Microsoft Office; HMIS experience is a plus.

Responsibilities

• Coordinate intake and eligibility for new clients.
• Provide outreach to homeless Veterans in the field.
• Maintain a caseload and conduct home visits as needed.
• Develop budgets and service plans with Veterans.
• Coordinate with landlords and housing specialists to sustain housing.
• Ensure HMIS data entry within 24 hours of service delivery.
