Job summary

Catholic Community Services in Delaware is seeking a full-time Case Manager to support Veterans and their families under the Supportive Services for Veteran Families program. You will conduct intakes, assess needs, and help secure housing using Housing First and Progressive Engagement approaches.

The role includes HMIS data entry, developing Housing Stability Plans, and coordinating with community resources to achieve rapid rehousing. Competitive benefits accompany a 40-hour work week.

Qualifications

• Post-secondary education in a social services field and a minimum of 1 year experience.
• Experience serving low-income households and connecting to housing and support services.
• Ability to work independently as well as in a team.
• Strong self-motivation with ability to manage multiple tasks in a high-pressure setting.
• Excellent interpersonal, oral and written communication skills.
• Strong organizational and time management skills with thorough record-keeping.
• Computer skills including Microsoft Word and database experience.
• Reliable transportation and valid Washington driver’s license (per CCS policy).
• Upon employment, obtain TB test and CPR/First Aid training.
• Criminal history background checks required.
• Fluency in Spanish preferred.

Responsibilities

• Receive referrals from Coordinated Entry.
• Screen clients for Rapid Resolution.
• Screen households using standardized assessment tools.
• Complete intake and determine eligibility with referred clients.
• Develop and initiate Housing Stability Plans.
• Assist with locating and securing housing.
• Input data into HMIS and maintain records.
• Attend weekly meetings and engage with community resources.
