The Salvation Army in the United States seeks a Case Manager for the Supportive Services for Veteran Families (SSVF) program. You will provide comprehensive case management and supportive services to Veteran households, delivering rapid resolution, rapid rehousing and homelessness prevention as needed.

Responsibilities include onboarding staff, engaging with veterans and community partners, coordinating with VA, CoC and other agencies, and documenting progress in HMIS.

Qualificações

• Bachelor’s degree required; social work preferred.
• 2–5 years of relevant experience; experience with motivational interviewing, solution-focused interventions, and mediation.
• Proficiency in Motivational Interviewing, Crisis Intervention, and Trauma-Informed approaches.
• Ability to operate telephone and electronic communication devices and basic office equipment.
• Demonstrated case management skills including outreach, engagement, and documentation.

Responsabilidades

• Provide case management and supportive services to Veteran households under SSVF.
• Assist with staff training and onboarding of new personnel.
• Engage with Veteran households, referrals, landlord outreach, and community networking.
• Screen applicants within 24–48 hours; provide referrals for those eligible and not eligible.
• Conduct intake/assessment and participant orientation.
• Develop housing stability plans and coordinate services with VA, CoC, and others.
• Maintain HMIS data and participate in CQI projects.
