Job summary

Endeavors in Tucson, AZ is seeking a dedicated Case Manager to support homeless Veterans in the Homeless Veteran Reintegration Program (HVRP). You will provide comprehensive case management, using trauma-informed care and a strengths-based approach to help participants gain stable employment and housing.

You will build relationships, conduct assessments, coordinate resources, document in HMIS, and collaborate with a multidisciplinary team to promote independence and lasting outcomes.

Qualifications

• Bachelor's degree in social work, public administration, psychology, or related field.
• 1+ years in a social service setting; experience with veterans is a plus.
• Valid driver's license with clean record.
• Must have daily use of a vehicle and be willing to travel.
• In-office location in Tucson, AZ.

Responsibilities

• Develops effective, trusting relationships with program participants using trauma-informed care and a strengths-based approach.
• Administers assessments to determine knowledge, skills and abilities; identifies barriers to employment and develops an action plan for placement.
• Advocates for housing and community resources, including school programs, vocational opportunities, and mental health services.
• Maintains outreach with landlords and partners to support housing stability for participants.
• Documents weekly case notes, HMIS entries, and incident reports; coordinates with multi-disciplinary teams.
