Job summary

Endeavors is seeking a dedicated veteran-focused case manager in Texas to support prevention of homelessness. You will conduct eligibility screenings, coordinate housing referrals, and connect Veterans and families to services, with an emphasis on housing stability and self-sufficiency.

The role involves outreach in shelters and community sites, liaison work with landlords, and ongoing documentation of progress. Occasional travel, nights and weekends may be required to meet program needs.

Qualifications

• Bachelor’s degree in Social Work, Sociology, Psychology or related field required.
• 1+ year of case management experience; 3+ years preferred.
• 2+ years in a customer service focused environment.
• Experience with homeless, low income, veterans & their families a strong plus.

Responsibilities

• Advocate for Veterans and facilitate access to housing and other benefits.
• Develop and maintain a comprehensive database of community resources and referrals.
• Conduct outreach to shelters, encampments, and stand-down events.
• Deliver presentations to community organizations and participate in Veteran-focused meetings and program activities.
• Complete intake assessments, comprehensive needs evaluations, and individualized service plans.
• Coordinate, monitor, and document services toward established goals.
• Assist Veterans in locating, securing, and maintaining housing; build relationships with landlords.
• Must be available for travel, nights, weekends and holidays as required.
