Job summary

U.S.VETS Phoenix seeks a Case Manager II to provide trauma-informed, strengths-based case management to veterans facing housing instability, trauma, and other barriers to stability. You will assess needs and develop individualized service plans addressing housing, employment, education, recovery, and health challenges.

You will coordinate with the VA, landlords, employers, treatment providers, and community resources; conduct home visits; and support group work for diverse veteran populations,

Qualifications

  • Bachelor's degree in social work, psychology, counseling, or related field.
  • At least two years of work experience in case management or related social services.
  • Ability to pass background checks for programs involving incarcerated or recently released clients.
  • Strong communication, documentation, and organizational skills.
  • Proficiency in Microsoft Office and electronic records systems.
  • Valid driver's license with clean driving record; able to complete driver training.
  • Experience with homeless, veteran, or justice-involved populations (preferred).
  • Certification in Substance Abuse Counseling (e.g., CCAPP, CAADE).
  • Experience facilitating groups or life skills classes.

Responsibilities

  • Conduct thorough client assessments and develop individualized service plans.
  • Coordinate and monitor services with the VA, landlords, employers, treatment providers, and community resources.
  • Provide regular one-on-one meetings and home visits for housing programs.
  • Document all services and critical incident reports in a timely and professional fashion using MCR and other electronic records systems.
  • Conduct random drug and alcohol screenings and ensure compliance with program rules.
  • Support reporting, audits, and file reviews in collaboration with the Program Coordinator and Director of Behavioral Health.
