Veterans Multi-Service Center, Inc. in Lock Haven, PA, seeks an HVRP Case Manager to provide trauma-informed advocacy and comprehensive housing-focused case management for 15–30 Veteran households, guiding families from housing crisis to stable housing.

You will collaborate with interdisciplinary teams, assess eligibility, develop individualized housing stability plans, and coordinate access to VA benefits and other public programs, while maintaining compliant client files and supporting

Qualifications

• Bachelor’s degree or equivalent work experience in social services, non-profit management, administrative or case management capacity.
• Two years’ experience in non-profit, social services, or related program delivery and coordination.
• Experience with grant-funded programs, especially veteran housing services, with diverse populations.
• Good computer skills with knowledge of databases and Microsoft Word Suite.
• Strong organizational, time-management, communication, and people skills.
• Valid driver's license with acceptable driving record.

Responsabilités

• Maintains a caseload and provides case management with a focus on housing first.
• Consults with clients to assess program eligibility and service needs.
• Creates individualized housing stability plans with client-specific goals.
• Provides ongoing assessment, planning, and implementation of program resources and services.
• Collaborates with interdisciplinary teams to coordinate service plans.
• Ensures case files comply with regulatory standards and confidentiality policies.
• Organizes outreach to meet requirements of VA benefits access and public benefits.
• Provides financial assistance to eligible households as needed.
• Develops relationships to generate referrals for veteran households.
• Attends internal, external and community meetings and events.
• Performs additional program-related duties as assigned by the Program Manager.
• Maintains consistent attendance and supports department operations.
