Job summary

Endeavors in San Antonio, TX seeks a dedicated Case Manager to provide comprehensive support to Veterans and their families who are homeless or at risk. You will advocate for access to housing, utilities and benefits, and build networks of community resources to strengthen client outcomes.

Travel up to 30% to various locations, maintain accurate records, and collaborate with the Lead Case Manager to improve program effectiveness.

Qualifications

• 1+ years of case management experience; 3+ years preferred.
• 2+ years of customer service experience.
• LMSW/LBSW/LMFT preferred.
• Bachelor’s degree in the listed fields required.

Responsibilities

• Advocate for and facilitate Veterans’ access to housing, utilities, and relevant community services and benefits.
• Build and maintain a database of community resources.
• Conduct initial intake meeting with Veteran and evaluate needs; develop service plan.
• Coordinate and monitor services; track Veteran activities and housing inspections.
• Prepare reports and coordinate payments with the Financial Assistance Coordinator; staff with Lead Case Manager.
• Travel up to 30% within county and surrounding areas; participate in community groups and provide presentations.
