Job summary

Heading Home seeks a Case Manager I - HVRP in Albuquerque to support veterans facing homelessness in gaining employment and stable housing. You will guide intake, skill training, job placement, and ongoing career stabilization through a coordinated plan.

Responsibilities include community outreach, eligibility assessments, IEP development, service coordination with VA and housing programs, and post-placement follow-up to ensure retention. Strong local knowledge and driven advocacy are essential.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Counseling, or Human Services is preferred; direct experience may substitute for a degree.
• Minimum 1–2 years in human services, career counseling, or case management.
• Familiarity with Albuquerque-area resources and veteran-specific aid networks.
• Proficiency with Microsoft Office and electronic case management tracking systems.

Responsibilities

• Conduct active outreach at local shelters, Stand Down events, and community locations to connect with eligible veterans.
• Evaluate program eligibility and identify employment barriers and housing needs.
• Work one-on-one to build an Individualized Employment Plan (IEP) outlining career goals, resume building, and interview prep.
• Coordinate with VA clinics, housing agencies, and substance use treatment programs to link services.
• Assist with local job searches, coordinate vocational training, and maintain contact for up to nine months after placement.
