MACV Minnesota Assistance Council for Veterans in St. Paul seeks an Intake Coordinator to serve as a primary contact for Veterans and community partners, assess eligibility, gather information, and coordinate referrals to MACV services.

The ideal candidate will communicate professionally, stay organized under pressure, manage multiple referrals, maintain confidential records, and provide administrative support to the Northern Region as needed.

Qualifikationen

• Experience with HMIS or other homeless services/client information systems.
• Knowledge of Veteran services, Veteran issues, or community-based resources.
• Experience working in a human services, nonprofit, healthcare, social services, or similar environment.
• Experience coordinating referrals or navigating individuals through multiple service systems.
• Familiarity with local community resources and service providers.
• Experience working with individuals experiencing housing instability or other significant barriers to stability.
• Valid Driver's License

Aufgaben

• Conduct initial screening and assess Veteran eligibility for MACV services.
• Review information and determine next steps; connect Veterans with MACV or other resources.
• Process and forward eligible referrals to case management staff.
• Maintain accurate documentation in shared databases and client tracking systems.
• Communicate eligibility determinations and referral outcomes to Veterans and partners.
• Obtain additional documentation as needed to facilitate timely assistance.
• Respond professionally to Veterans, providers, and stakeholders in urgent situations.
• Identify barriers and determine appropriate next steps.
• Manage multiple referrals and admin tasks with accuracy.
• Maintain client confidentiality per MACV policies.
• Provide administrative support to Northern Region as needed.
