Job summary

Minnesota Assistance Council for Veterans (MACV) seeks a JIV Case Manager to deliver housing-focused case management and reentry support to Veterans with justice-system barriers. The role collaborates with MACV housing teams, correctional agencies, VA programs, and community partners to promote housing stability and successful community reintegration.

The position requires coordination across facilities, courts, and community sites, with statewide travel and partial hybrid work.

Qualifications

• Bachelor's degree or equivalent experience in social services, justice, or related field.
• Experience with Veterans, justice-involved individuals, homelessness or reentry services preferred.
• Proficiency with Microsoft Office; ability to learn Salesforce and other case-management systems.
• Strong communication, organization, documentation, problem-solving, and relationship-building skills.
• Comfort working with Veterans charged with or convicted of serious offenses; travel throughout Minnesota.

Responsibilities

• Conduct regular outreach with correctional facilities to identify veterans at risk of homelessness upon release.
• Collaborate with MACV programs and community partners to develop individualized reentry plans.
• Complete client intakes including documentation, screening, assessments, and orientation.
• Identify barriers affecting housing and develop housing and reentry goals with the Veteran.
• Coordinate pre-release and community reentry planning with correctional staff, VA programs, and MACV teams.
• Maintain timely case notes and documentation in Salesforce and related systems.
• Communicate major barriers and safety concerns to the JIV Program Director.
• Maintain a post-release caseload focusing on housing stability and community transition.
