Job summary

WestCare Foundation is seeking a Senior Case Manager in Fresno to manage a caseload of low-income veteran participants. You will perform case management, link veterans with VA and community services, and emphasize Housing First principles to help achieve housing stability.

Responsibilities include assessments, referrals, and coordination with community providers, landlords, and service teams. A strong focus on participant-centered practices and empathy is essential.

Qualifications

• One year of case management experience is preferred.
• High school diploma or equivalent required; two-year college degree preferred.
• Ability to document clearly in standard county forms and systems.
• Experience with connecting clients to community and VA services.

Responsibilities

• Serve as part of the SJVV case management team and perform required duties.
• Support a large caseload of homeless veterans or those at risk.
• Create and monitor individual plans, update weekly, and set exit plans.
• Coordinate referrals to programs and facilities for eligible veterans.
• Educate veterans about food, clothing, employment, and transportation resources.
• Advise on community resources and establish support networks.
