Job summary

Insight Housing in the Bay Area is seeking an Outreach Specialist to support the Roads Home team by engaging Veterans experiencing homelessness, building rapport on the streets, and connecting participants with housing and case management services.

You will assess eligibility, complete intake, coordinate with mental health, substance use, and VA benefits, document on HMIS, and promote safe, professional outreach in collaboration with local agencies.

Qualifications

• High school preferred.
• Ability to complete CPR/First Aid Training.
• Prior direct support experience with Veterans and individuals experiencing homelessness, mental health, substance abuse or other social issues preferred.
• Proficient in Microsoft Office and HMIS is a plus.

Responsibilities

• Provide street-based outreach to veterans and individuals facing homelessness in service areas.
• Serve as liaison between homeless individuals and Roads Home team.
• Conduct outreach using active listening and motivational interviewing techniques.
• Assist with enrollment, intake and connecting participants to housing and case management services.
• Document services accurately in HMIS and adhere to HIPAA standards.
