Job summary

Endeavors in Tucson, AZ seeks a Case Manager to provide comprehensive case management to homeless Veterans in the HVRP, using trauma-informed and strengths-based approaches to help them achieve stable careers.

Responsibilities include building trust, conducting assessments, identifying barriers, developing goals, coordinating housing, services, and employment resources, and documenting in HMIS.

Qualifications

• Bachelor’s degree in Social Work, Public Administration, Psychology, or related field.
• 1+ years of experience in a social service setting or customer service.
• Driver’s license with a clear record and daily vehicle use; ability to travel.

Responsibilities

• Develops trusting relationships with participants using trauma-informed and strength-based approaches.
• Administers assessments to determine knowledge, skills and abilities; identify barriers to employment.
• Uses Housing First model to access housing resources and community services.
• Maintains outreach with landlords and community partners to secure housing.
• Completes service plans with goals and follows up on progress.
• Documents case notes and HMIS records; prepares incident reports.
• Maintains communication with multidisciplinary teams across programs.
• Facilitates grant-approved training opportunities for participants.
• Provides life skills training and budgeting support.
• Performs other duties as assigned.
• Demonstrates exceptional customer service to empower participants and their families.
