Job summary

The Transition House, Inc. is seeking a compassionate Veteran Case Manager to provide community-based, non-clinical suicide-prevention case management through our Veteran Connection and Suicide Prevention Program.

You will help Veterans reduce barriers, navigate VA and community systems, and stay connected to individualized services and supports, including housing stability, benefits, healthcare, and transportation. Strong coordination and documentation are essential.

Qualifications

• Bachelor's degree in social work, human services, psychology, or related field, or equivalent experience.
• Experience providing case management, care coordination, outreach, resource navigation, or social services.

Responsibilities

• Engage, assess and plan with an assigned caseload based on risk and goals.
• Coordinate services and warm handoffs to VA and community resources.
• Identify risk, document, and escalate when there is imminent danger.
• Engage families and supports in service planning and education on suicide prevention.
• Document notes, referrals, and client progress in secure systems.
