Job summary

WestCare Georgia Inc. is seeking a Case Manager to provide intensive, person-centered case management for veterans and service members identified as at risk for suicide.

You will assess needs, develop service plans, coordinate care, and connect clients with behavioral health, housing, and employment resources in a collaborative team. Responsibilities include ongoing care coordination, advocacy, and timely documentation, while maintaining confidentiality and adhering to program protocols.

Qualifications

• Two years of experience in case management, behavioral health, veteran services, or a closely related area.
• Experience working with high-risk populations including individuals with mental health or substance use concerns.
• Knowledge of suicide prevention principles, trauma-informed care, and community resources.

Responsibilities

• Conduct comprehensive assessments to identify participant strengths, needs, risks, barriers, and service priorities.
• Develop, implement, monitor, and update individualized service plans with participants and the program team.
• Provide ongoing case management, care coordination, advocacy, follow-up, and resource navigation for veterans at risk for suicide.
• Connect participants to behavioral health, housing, employment, benefits, transportation, and other community resources.
• Maintain confidentiality and complete accurate documentation within required timeframes.
